Adult Services

Adult Community Support Services

Community Support Services (CSS) provides a variety of supports and services designed to help adults living with a severe and persistent mental illness (SPMI). This program offers effective, integrated, and coordinated treatment within the community to assist individuals managing conditions such as major depression, bipolar disorder, or schizophrenia.

For many, these conditions are serious and may have resulted in hospitalization. CSS helps individuals increase their ability to live independently, develop and maintain employment skills, build social connections and a sense of community, and reduce the likelihood of hospitalization.

Services are designed for convenience, with support available in the home, community, or even the workplace. Each individual takes an active role in their treatment—identifying personal goals, focusing on strengths, and collaborating with their treatment team to create an individualized plan that promotes recovery and well-being.

Case Management

Case management is a medically necessary, one-on-one service. Most case management services are provided in the person’s home or community. Case managers coordinate care among treatment providers and follow the medically necessary treatment plan. Some goals a case manager may work on with an individual include symptom reduction, building coping skills, promoting wellness, developing healthy relationships, and strengthening independent living skills such as financial management, nutrition, self-care, and recreation. 

Attendant Care

Attendant care is an additional service that provides friendly support for everyday functioning. Attendant care services enable individuals to accomplish tasks or engage in activities they would normally perform themselves if they did not have behavioral health challenges. Examples of assistance may include supervision to maintain safety, ensuring that basic needs are met and promoting independence. 

Peer Support

Peer Support is a person-centered service with a focus on restoring the individual to their previous level of functioning. Services can be one-on-one or in a group. It is designed to help with managing and coping with psychiatric or substance use symptoms using community resources. Providers are self-identified recipients of behavioral health services. Peer support emphasizes the importance of supporting one another and taking full advantage of “lived experiences” as valuable resources to be used on the road to recovery. 

Housing Program

The goal of this program is to increase capacity and provide accessible, effective, comprehensive, coordinated, integrated, and evidence-based treatment services, peer support, and other recovery support services for individuals with a serious mental illness, who are experiencing homelessness. The program should also provide linkages to health insurance, Medicaid, mainstream benefits and sustainable permanent housing.
