Community Based Services (CBS)

What is CBS?

Community based services are designed to provide children who experience serious emotional disturbances (SED) with supports to help the child be able to maintain functioning in the community and home setting.

Some of the services that your child or family may be eligible for are:

  • Case Management 
  • Target Case Management 
  • Attendant Care
  • Parent Support
  • Crisis Services 
  • Psychosocial Rehabilitation

Who Is Eligible?

Children must meet eligibility criteria in order to receive community based services. A qualified mental health professional must assess the child’s functional abilities and deficits and deem the services medically necessary. 

Our Services

Parent Support: A parent support worker provides support and training in regards to your child’s mental health diagnosis. They may assist you in parenting strategies, coping skills, and understanding the mental health system. 


Psychosocial Rehabilitation: This service may be provided in a group or individual setting. Staff will assist children in learning life skills in a goal directed setting. Examples: self care, social skills, or problem solving. 


Case Management:  This is a goal directed service providing supports and interventions directly to your child. It is a face to face service, often provided in home, intended to help your child achieve goals that your family has helped to set. 


Targeted Case Management: TCM Care Coordination is a service that provides a single point of contact through whom services and resources may be accessed. A provider will coordinate all services. 


Attendant Care:  This service is provided to Medicaid eligible children to support, supervise, and cue them. This enables children to accomplish tasks in the home, school or community. 


Crisis Services: Crisis services are provided to children who are experiencing a mental or behavioral crisis. Services, can be accessed 24 hours a day, seven days a week. 



The Home & Community Based Waiver

 Children who are experiencing serious emotional disturbance and who are determined as likely to need state hospital level or care may qualify for the SED waiver services to help them maintain functioning in the home and community. In addition to the services listed inside, waiver services provided may include:

  • Independent living skills
  • Respite care (if not in foster care)
  • Professional Resource Family Care (not in foster care)
  • Wraparound Facility.
